The nature of injuries in physical activity
Physical activity and sport always involve some risk of injury, although serious ones are very rare. Most injuries happen when blood vessels are damaged, leading to bleeding that can be external or internal. This damage often causes swelling and pain as the body's natural response to protect the area and start healing.
Key types of tissue and injury severity
- Soft tissue - The softer parts of the body, including tendons (which connect muscles to bones), ligaments (which connect bones to bones), skin, fat, muscles, nerves, and blood vessels. Hard tissue refers to bones and teeth.
- Minor injuries - These are less serious issues, like cuts, bruises, or grazes, that can usually be handled on-site by a first aider without needing professional medical help.
Common minor soft tissue injuries
Many injuries in sport are minor and affect soft tissues, often from impacts, friction, or overuse. These can usually be managed with simple treatments to reduce pain and promote healing.
Blisters
Blisters form as a protective response when skin is damaged, creating a small pocket of fluid in the upper layers.
Characteristics of blisters:
- Description - A bubble-like area filled with clear fluid called serum, though it can contain blood or pus if the blister becomes inflamed or infected.
- Cause - Repeated rubbing against the skin, such as from ill-fitting shoes, or from burns.
- Treatment - For unbroken blisters, avoid piercing them to prevent infection and cover with a plaster or dressing. For burst blisters, do not peel off the dead skin; let the fluid drain naturally, clean the area with mild soap and water, then apply a sterile dressing.
Bruises
Bruises occur when small blood vessels break, allowing blood to leak into surrounding tissues.
Characteristics of bruises:
- Description - Patches of skin that appear bluish or purple due to blood from broken capillaries (tiny blood vessels) seeping into soft tissue.
- Cause - A direct impact with a hard object, like being hit by a ball or colliding with equipment.
- Treatment - Use cold compresses to help reduce swelling and limit internal bleeding; apply them gently to the area.
Cuts and grazes
These injuries involve breaks in the skin that damage underlying blood vessels, leading to bleeding.
Characteristics of cuts and grazes:
- Cuts - Deeper breaks in the skin where blood vessels beneath are damaged.
- Grazes - Surface-level damage where skin is rubbed off, exposing and harming blood vessels.
- Causes - Cuts often result from contact with sharp objects, while grazes come from scraping against rough surfaces.
- Treatment - Apply pressure using clean material to stop the bleeding, then clean the wound thoroughly and cover it with a dressing to protect against infection.
Winding
Winding is a temporary breathing issue that can feel alarming but usually resolves quickly.
Characteristics of winding:
- Description - Sudden difficulty in breathing, often after intense exertion or a blow to the stomach.
- Cause - A direct hit to the abdomen that causes the diaphragm (the muscle separating the chest from the abdomen) to spasm.
- Treatment - Loosen any tight clothing, sit in a crouched position to help relax the muscles, stay calm, and take slow, deep breaths until normal breathing returns.
Muscle, tendon, ligament, and joint injuries
Some injuries affect the structures that support movement, such as muscles, tendons, and ligaments. These often stem from overuse or sudden forces and can range from mild to severe, sometimes requiring more than basic first aid.
Muscular injuries (strains)
- Description - Damage where a muscle is overstretched or partially torn, commonly called a strain.
- Cause - Overuse of the muscle during physical activity, frequently affecting the legs (like hamstring strains) or back (lumbar strains).
- Treatment - Follow the RICE method (detailed later), and limit training to allow recovery.
Tendon injuries
- Description - Overstretching or tearing of tendons, or inflammation known as tendonitis from repeated stress.
- Cause - Sudden sharp movements or ongoing overuse.
- Treatment - Use the RICE method, reduce training intensity, rest the area, and take painkillers if necessary to manage discomfort.
Ligament injuries (sprains)
- Description - Twisting or pulling of ligaments beyond their normal range, also known as a sprain; severe cases like anterior cruciate ligament (ACL) tears are common in sports involving quick direction changes, such as football or basketball.
- Cause - Forcing a joint into an unnatural position.
- Treatment - Start with the RICE method; if there's severe pain or inability to move, seek medical help, as surgery might be needed for serious tears.
Joint dislocations
- Description - A joint is pushed out of its proper position by sudden force, causing swelling and intense discomfort.
- Cause - High-impact forces, such as falls or collisions in contact sports.
- Treatment - Apply the RICE method immediately and get to a hospital for professional care; in some cases, the joint can be relocated by a medical expert.
The RICE method for treating injuries
The RICE method is a standard first-aid approach for many soft tissue injuries, helping to reduce swelling, pain, and further damage. It stands for Rest, Ice, Compression, and Elevation, and should be used soon after the injury occurs.
Steps of the RICE method
- Rest - Stop all activity immediately to protect the injured area and prevent worsening the damage.
- Ice - Apply an ice pack wrapped in a towel for 15-20 minutes every 2-3 hours; this helps numb pain and constrict blood vessels to limit swelling.
- Compression - Wrap the area with an elastic bandage to apply gentle pressure, which decreases swelling but should not be too tight to avoid cutting off circulation.
- Elevation - Raise the injured body part above the level of the heart whenever possible, as this uses gravity to drain excess fluid and reduce swelling.
